In addition to the ability to apply for up to 3 months of coverage, members in the following states have the option at the time of enrollment to apply for a second consecutive policy for up to 3 months of coverage. AL, AZ, AR, DE, DC, FL, GA, IL, IN, IA, KY, LA, MS, MO, NE, OH, OK, SC, TX, VA, WI.

How does this coverage work?*

When up to 3 months of coverage is selected and members opt to apply for a second policy at the time of enrollment, they will be issued 2 separate policies. Once the first policy expires, they will receive:

  • A new member ID number and card
  • A new policy certificate that resets the benefit terms
  • A new policy with 100% coinsurance
  • Deductible will be waived on 2nd coverage period
  • A new maximum out-of-pocket amount
  • A new maximum limit

Why should members apply for a second consecutive policy at the time of enrollment?

By choosing this option, members can apply for a pre-existing conditions waiver. This will waive any conditions that were covered during the prior coverage period (i.e. during the first up to 3 month policy).Pre-existing condition waiver rider availability varies by state.

What is the waiver of pre-existing conditions?*

The pre-existing condition limitation will not apply to any conditions that were covered during the prior coverage period. Any other pre-existing conditions will remain subject to the limitation as described in the Certificate of Insurance.

Does the plan require Pre-Certification?*

All Inpatient hospitalizations and procedures done at an Outpatient Surgery Facility must be pre-certified. LifeShield National Insurance Co.’s professional review organization must be contacted as soon as possible before the expense is to be incurred. If the Covered Person does not comply with the Pre-certification requirements as stated in the insurance certificate, the Eligible Medical Expenses will be reduced by 50%..

Who is eligible to apply for this insurance?

LifeShield STM is available to members and their spouses, who are between 18 and 64 years old and their dependent unmarried children under 26 years old; and can answer "No" to all of the questions in the application for insurance. Child-only coverage is available for ages 2-25.
